HOW TO GET A PHILHEALTH CARD: A Card that Saves Lives

• Fill out two (2) copies of the PhilHealth Member Registration Form which you can get at: https://www.dlsu.edu.ph/offices/accounting/_pdf/pmrf.pdf

• Submit PMRF to the nearest PhilHealth stations with the documents listed at https://www.philhealth.gov.ph/members/lifetime/registration.html

Steps to register PhilHealth online:

• First is go to the website at https://eregister.philhealth.gov.ph 

• In the site, you will see the other system registration and after you read all of it, click the proceed button. 

• The next step is to agree to all the Terms and conditions. 

• After reading and agreeing in the terms and conditions, click I agree box and accept button. 

• The next thing to do is to fill up the form.

1. The first part of the form is personal information and should be completely filled up. 
2. Next is contact details, complete all the data needed to continue with the registration. 
3. After that, fill up the address form then the dependents information.
4. Then, fill up as well the guardian information or the one who will be contacted in case emergency.
5. The last part is the membership category which would ask for the category one opt to belong, the name of the employer if employed and profession description.

• After filling up the form and giving the needed information, one can now successfully submit the form to finish the registration and acquire the PhilHealth card from the nearest PhilHealth stations after payment. 

• There will be a prompt to save the information given and one will be notified registration to PhilHealth online is successful.
